Only Harbor-UCLA Medical Center and Los Angeles Biomedical Research Center Staff/Faculty/Residents may use these forms to request articles, book chapters and books.
Before using these forms to request document delivery, please check the Parlow Library Online Catalog and Parlow Library Online Journal List for your request. The Reference Department will only process requests for journals or books that are not in the Parlow Library collection.

- Copyright Law of the United States (Title 17, United States Code) governs reproduction of copyrighted material.
